Shifting into second
Pretty normal for lazy shifting.. No offense. What I mean is, 1st to 2nd is skipping over neutral, so it requires a bit firmer shift when compared to the other gears. A lot of times, you don't even realize your shifting too soft until it doesn't go into gear, or it pops back out.
If your riding in shoes, I'd suggest buying a set of riding boots. That can make a huge difference in your shifting. You will put more force into the shifter without more effort on your part as the boots have a much harder top surface that doesn't deflect like a pair of shoes will when shifting.

Man, I didn't even think of that. DUH. Have you adjusted your shifter height? I'm so used to troubleshooting broken stuff that I forget to address the basic setup questions.
I still think its a worn gear, though, as shifter height would affect ALL upper gears, not just second.

Also make sure you are preloading (putting pressure on the shifter) before you let off the gas or tap the clutch. I find that if I don't do this on my bike, I will miss the 1 -> 2 shift about a third of the time.
