2 suggestions really:
  1. Take the Motorcycle Safety Foundation's Basic Rider Course
  2. Pick up an excellent book: Twist of the Wrist 2 by Keith Code

Sounds like you need confidence. Educating yourself on riding builds confidence which will help buoy you until practice and skill takes over. The BRC lays an excellent foundation of both education and live practice. Keith Code's book deals with the details, explaining how and why things work. And helping to deal with the fears (He refers to them as S.Rs or Survival Reactions) newer riders face.

You might also want to check out the How To: Riding Skills section. There is a load of information in those sections from the members here that will help you improve your riding.
