Default Pads and rotors

When I'm replacing pads, should I resurface somehow (or turn them like shops do with car disc brakes? Likely don't grind them, but any kind of surface touchups or whatever?
AND what if the brakes still being used 'carefully' when heading a sounding carving/grinding sound. Do the rotors get damaged, if so, fixable too?

RE: resurfacing the rotors... The only thing you might want to do is scrub them thoroughly with a Scotch brite pad and some brake cleaner. If you notice any type of damage to the rotor while doing this, you should strongly consider finding out what is happening and why. A carving/grinding sound isn't something you want to ignore. Keep in mind though that the pads will sound like that it if they are lightly against the rotor. I was freaked out the first time I noticed it but that is normal.
