Default Bar end constant problem

I've notice that my right bar end keeps coming loose, and when I tighten it up it only goes so far before spining loose u can say. After tighting up it might last a month before its loose and bout to fall off. Is the problem the screw or the frame where it goes into? Would a lil bit of cheap glue holdit in place, or that white tape thats used in pipe fittings? Anyone had this problem before, any solutions??

I can only tighten it up so much, then it just spins loose again . . .

better than that white thraed tape, it will hopefully keep the threads from backing off and loosening up.
