When you get the rear wheel off, check to see if there is a lot of play in the rear sprocket assembly. It should move ever so slightly, but not wobble around a whole lot. I have not had to replace any on my bikes. Have 39K on one of them.

Mine failed but it was very obvious, the rubber connecter betwen the two pieces snapped. I had three of them go. By the way, there is a back order on those damn things... looking l.i.k.e I won't able to get them until September. I have a little over 50K on them.

Cush drive is just an abbreviation of Cushion drive, especially on a chain driven bike, no adjustment is 100% accurate, the cushioning is there to compensate for slight mis-alignments. And dampens the effect between the components of the rear wheel (sprocket, bearings & wheel hub) V's Horsepower, their life is shortened by rear wheels being out of whack.
