Chain Tensioner

Recently while riding I noticed that my chain tensioner was not pressed tightly into the end of the swingarm like it should be. I did what I could on the road and tightened up the nut and locknut. I got home and checked my alignment which was still dead on and I thightened both locknuts down pretty good. Tonight while riding I noticed that half of the same tensioner that was loose was now broken and half missing. How did this happen? How can I prevent it from happening again? I know that my axle nut is torqued to Honda regulation and my alignment has not changed, so where did the force to break my tensioner come from?
