Clutch shot?

Clutch shot?
Just double checking your alls opinion before I go and get this done, or tear her down and do it myself.
The bike runs fine, shifts smooth enough but; The engage point is at about 80% clutch lever released. When full throttle at about 9,000 RPM's it immediately shoots to red line. I can't verify the exact RPM's there as I try not to take my eyes off the road, but when getting on it it feels like the clutch is slipping. Time to slap a new one in?
