Front wheel bearings

Default Front wheel bearings

I just installed a set of front wheel bearings and 20 miles down the road the whole front end starts to get shaky so i pulled off the wheel and my right bearing half of the bearings were missing and the left bearing was locked up. I was thinking the bearings were not pressed in properly but i cant think of anything else.

Do you have the spacer tube installed that goes inside the wheel between the bearings? Something is putting sideways force onto the bearings. Incorrect tightening order?

yeah the spacer is installed. I made sure I aligned the forks before tightening everything down also. The forks are aligned also.
