Has anyone use this integrated tail/turn signal from Immortal Graphix?

That's the clear alternatives taillight, i have it and love it. Much brighter than stock, looks cool and is very easy to install. They're all over ebay for cheaper though.

If i get one, would I still need a fender eliminator or could i simply cut the old one off and make a tag holder?

Well not all FE kits come with blinkers, some come with just the plate mount to clean up that nasty rear fender!

clear alternatives and you need a license plate bracket probely find one on ebay for $30-40

i've got the same tail lights, I like em. Personally, I would put an undertale tag bracket on. IMO they are cleaner, stay on better, and are much easier to mount a light with.
