It would be pretty hard to unintentionally make a three way call on an iPhone. It has a proximity sensor that turns the screen off when it is up to your ear.
I had a crappy touch screen (HTC Touch) before the iPhone and it would do crap like that all the time.

Hell yeah I will stay on the line until they disconnect the call. Just put it on speaker and sit back and hope you hear something good.

havent had that problem with my voyager....I remember when cell phones didnt flip or anything, just a basic phone and I had problems with them calling people b/c I always forgot to lock the phone....
